Insurance and fee schedule:
We are a cash-only practice (we accept all forms of payments including personal checks, credit and debit cards, health savings account, etc.) as this allows us to maximize the amount of time we spend directly with our patients to meet their specific needs and avoid undue influence from insurance companies. After each visit, we are able to provide a "Superbill" for services rendered which can be submitted to the insurance company to get out-of-network reimbursement. Assistance with any additional forms, controlled-substance refills, telephone encounters or other services will be billed separately. Any cancellations within 48 business hours of the appointment or no shows will be charged the full cost of the appointment.
